MS FHIR version 4.0.728 · Overall: 97% (28/29 tests passed) on round 2026-q2-r000. Source: https://github.com/microsoft/fhir-server.
Microsoft FHIR Server (formerly known as the Azure API for FHIR OSS core) is a .NET-based open-source FHIR R4 implementation. It underpins Azure Health Data Services and is a common self-hosted alternative when teams want the Azure stack without the managed-service pricing.
Each cell links to the profile-level scoreboard comparing MS FHIR against every other tested server.
Tests are authored as FHIR R4 TestScript Resources and run by an open-source executor against a fresh Docker deployment of each server. We deliberately don't compute a composite number — read the methodology for the reasoning.
