About HAPI

HAPI FHIR is the open-source Java reference implementation maintained by Smile Digital Health. It's the most widely deployed FHIR R4 server in production — used by public HIEs, payer pilots, and the majority of healthcare startups that self-host FHIR.

Methodology

Tests are authored as FHIR R4 TestScript Resources and run by an open-source executor against a fresh Docker deployment of each server. We deliberately don't compute a composite number — read the methodology for the reasoning.
