The silent write drop

Requesting a write scope does not fail. It succeeds, and the granted scope set comes back without it. An integration that checks only for an error and not for the granted scopes will believe it can write, and will discover otherwise on the first POST at a customer site. Comparing requested against granted scopes is the test this reproduces.

Plainer than the others

MEDITECH is the least mutated of the three vendor shapes: standard terminology systems, no proprietary OID dual-coding, no preflight search rejections. That makes it a useful control — a difference that shows up against Epic or Oracle Health but not here is a vendor quirk, not a bug in your client.
