Provider-facing and patient-facing traffic reach different hosts with different capabilities. An integration built against one and shipped against the other fails on searches that simply are not offered, which is why the map reproduces both rather than averaging them.
Issuing Patient? with no qualifying parameter returns a rejection with a specific diagnostic string, not an empty Bundle. Clients that treat "no results" and "refused" identically will report zero patients to your user and log nothing. The verbatim diagnostic is reproduced so you can assert on it.
Oracle Health uses the SMART v2 granular scope suffixes. A client that requests the v1 read/write forms may be downgraded or refused depending on tier — see the SMART on FHIR guide for the syntax.
