mock.health is a synthetic EHR sandbox driven by a Markov progression module trained on millions of real patient journeys, served through HAPI FHIR R4 with a full SMART on FHIR authorization server. On top, a mutate-on-read layer turns one canonical patient set into Epic, Cerner, and Meditech personalities at the wire. Custom cohorts by morning.

Why this exists

Every health-tech team hits the same wall: the demo says TEST TEST, the Epic sandbox has eight patients, and the integration test suite is 400 lines of hand-crafted fixtures. Everyone rebuilds the test-data pipeline. Everyone waits months for EHR access. Everyone ships shallow.

What's under the hood

A Markov progression model trained on millions of de-identified patient journeys, served through a HAPI FHIR R4 instance with a full SMART on FHIR authorization server. It's the stack you'd build yourself if you had six months — or the patience to work through MEPS, MIMIC, and CMS BCDA surveys. On top, it's an API key and patients that don't embarrass you.

Three things I believe

Your demo is your company's face. You cannot close a health system deal on fixture data that says TEST TEST. Standards are the only way. FHIR conformance is required — see the conformance heatmap. Your cohort isn't in the catalog. When you need Neuro ICU patients with Glasgow Coma Scale scores and TCD velocities, you shouldn't wait on a product roadmap. Overnight, not quarters.
