Outlier Epic cluster: just 4 of 739 production endpoints share this CapabilityStatement shape, including ENDEAVOR HEALTH CLINICAL OPERATIONS. This cluster advertises 47 US Core 6.1 profiles in its CapabilityStatement. A long-tail deployment variant that integrators rarely see in vendor sandbox documentation.
A CapabilityStatement shape cluster groups production FHIR endpoints by the exact CapabilityStatement they serve at runtime. Epic publishes one reference CapabilityStatement, but production hospitals deploy several distinct shapes of it — each cluster is one of those shapes.

Observation.code on vital-signs uses LOINC codes outside the official us-core-vital-signs subset Vital-signs LOINC selection is wider than US Core's named subset — clients filtering by `code.coding.code IN us-core-vital-signs-VS` will reject conformant Epic responses. Match by `code.coding.system='http://loinc.org'` and validate semantically rather than against the closed subset.

This outlier cluster has too few production endpoints for us to host a dedicated mock under /v/cluster/epic-cluster-D/ today. The modal Epic cluster is the wire shape that most production deployments use — that's the right starting point for integration testing.
